personal health navigators
Your personal health navigator is your dedicated partner for aging in place. From day one, they take the time to understand your health needs, lifestyle preferences and long-term care goals. Through this process, they create a personalized and proactive plan for you that supports independent living at home. Your navigator coordinates all aspects of your healthcare, handling medical appointments, home care services, insurance tasks, and transitions between providers. In taking care of these functions for you, they make sure you get the care you need while reducing stress for you and your family.
Health Services
When you need assistance with activities of daily living (ADLs), your membership plan provides coverage for home care services, helping you remain safe and supported at home.
Our personal health navigators are here for you around the clock in the event of a healthcare emergency, providing reassurance and guidance when needs arise at any hour.
Each member is given an emergency response device that is monitored 24/7 in case of an unexpected medical event. The device can be worn comfortably by the member, is GPS-enabled, has a two-way communication system, and is equipped with fall detection.
Through our extensive partnerships, we can help you tap into extended community resources that can help you gain access to additional care and support services.
Wellness Services
Your personal health navigator will conduct a comprehensive assessment to create a tailored wellness plan that supports healthy aging, one that can evolve as your needs and preferences change.
As a Friendship at Home member, you can access personal training services at Friendship Village of Dublin, helping you to maintain strength, balance and energy.
You can enjoy the wellness amenities at Friendship Village of Dublin, including exercise classes, fitness equipment, Olympic-sized pool, dining rooms, and social programming.
Lifestyle Services
Licensed occupational or physical therapists conduct home safety assessments to identify potential hazards and recommend modifications that support safe aging in place.
Members are invited to social gatherings, educational programs, and special events, fostering connection and community.
Through our partnership with Dorvie, a national lifestyle concierge service, members can have access to home repair, cleaning, laundry, and other household support services. These fees are paid directly by the member to Dorvie.
Meal delivery services are available through our plans when they are deemed medically necessary during a recovery period. Ongoing meal delivery services are available through our concierge partnership with Dorvie, providing convenient, nutritious options when needed. These fees are paid directly by the member to Dorvie.
Transportation is available through our plans for qualifying medical appointments and care-related needs. Everyday transportation services are available through Friendship Village or our concierge partnership with Dorvie. Fees associated with Dorvie services are paid by the member directly to Dorvie.
Coverage That Goes With You
Your Friendship at Home membership is designed to support aging in place, wherever life takes you. If you relocate outside the Friendship at Home service area, your plan benefits move with you. Your personal health navigator will remain available 24/7 to coordinate healthcare services and arrange care at home as needed. If you choose to move into a senior living community in another location, your care coverage benefits can be applied toward assisted living or skilled nursing costs, just as they would at Friendship Village of Dublin.

Friendship at Home offers four membership plans with varying levels of lifelong care coverage, allowing you to choose the option that best supports your aging in place goals. Each plan includes healthcare navigation, wellness services, annual assessments and lifestyle support programs. For covered care services, each plan covers a specific percentage of the actual Daily Care Charges or the Daily Maximum Benefit (DMB).
