Every Friendship at Home member is paired with a dedicated personal health navigator and support team who provide ongoing guidance—supporting wellness and prevention, coordinating healthcare and insurance needs, and managing transitional care during emergencies, hospitalization, rehabilitation or recovery at home.

Aging well requires access to a wide range of health, wellness and lifestyle services. Your membership includes access to services and coverage options that support independent living at home. For example, you may receive help managing your nutrition, exercise or medication. You can also access home care services or find assistance with meals, transportation or home maintenance.
