When you become a member, you are provided with a professional personal health navigator, who specializes in the area of aging. The personal health navigator will make sure your home needs are met. As situations arise your personal health navigator will be available to help you navigate through these events. They coordinate things such as:
When appropriate, Friendship At Home will provide an Emergency Response System with 24-hour coverage and monitoring by a contracted provider. The member will have a choice between an at-home unit or a unit with built-in GPS capabilities. Your personal health navigator will help determine which unit best fits your needs.
Our team performs an in-depth assessment of your home to ensure it has the least amount of risk possible. We make recommendations for home modifications that minimize the risk of falls and promote safety while you remain independent in your home.
